Documents Required for Registration
1. Birth Certificate or Passport
2. Transfer card from school attended
3. Copy of latest standardized test scores (NJAsk or Terra Nova)
4. Health records/immunizations (including Mantoux - TB Screening and
3 Hepatitis B shots). A physical within the year of starting the Middle School.
5. Proof of residency (lease to apartment or deed/tax bill for home)
6. Identification of person registering student
7. Final Report Card showing promotion to the next grade
8. Special Education students: CST needs a copy of the child's current I.E.P.
9. 2 contacts for emergency purposes...... names and phone numbers.
AN APPOINTMENT IS NECESSARY IN ORDER TO ENTER A NEW STUDENT
